Recommended for any potential club member or visitor who is under the age of 18. Have a Parent or guardian complete this form before the youth plays.

BACKGROUND CHECK FORM

Some club like to run a background check on potential members. Have them fill out this form and forward it to your local Firearms Officer.

LIABILITY WAIVER FORM

Have each member or visiting player sign this form prior to an event. A Liability Waiver is not as good as Liability Insurance but it does offer some piece-of-mind and lets player know what is expected

Use this form to keep track of the money being paid at your game days or other events. You can include your own rates so game day organisers know what to charge different types of players
